If you’re looking to cancel any membership or service agreement at any Tangible Wellness location, complete this form. Upon submission, you’ll receive a confirmation email and it will be process within 72 hours.

Fitness Center General Consent Form

All members or visitors using day passes, free passes or other services at our Fitness Center location are required to fill out this form. This form is provided in person and can also be submitted before your first visit.

Any individual wanted to utilize any of our MedSpa services or treatments must first review and sign this consent form. This form is provided in person or digitally before any treatment.
